Where do I send the medical form?

We are often asked "Where am I supposed to send the medical form?" Please note that this information is on the form itself.

At the very top of the form, on the very first page, you will see in large blue letters the email address medform@nasarallysport.com . This is the preferred method of submitting the form. You can either send a scan, or crisp, clear, high resolution photographs.
